To help people with Type 1 and Type 2 diabetes explore their health needs and understand and manage their diabetes in the best possible way.

Who we do it for

Diabetes Care for You is a service for adults living with Type 1 and Type 2 diabetes and registered with a GP practice in the Brighton & Hove and High Wealds Lewes and Havens area of East Sussex.

We help to deliver the Diabetes Care For You service in partnership with Sussex Community Foundation Trust.

Diabetes Care for You is a service for adults living with Type 1 and Type 2 diabetes and registered with a GP practice in the Brighton & Hove and High Wealds Lewes and Havens area of East Sussex. This service helps people to live well with diabetes by offering advice, support and helping with diabetes management and meeting personalised goals.

The team brings together consultants, diabetes specialist nurses, dieticians, podiatrists and psychologists, all of whom specialise in care for people with diabetes. We are a multidisciplinary team working across five sites in the Brighton & Hove, and High Wealds Lewes and Havens area of East Sussex

How can I access the service?

Referrals are usually made by your GP to this service, but the service can also be accessed through a referral from a hospital team or other healthcare professional.
