The Primary Care Collaboration team leads and supports projects within Primary Care. It’s important to us that we look at what’s important to the people we serve and find creative solutions to the question ‘what matters to you?’.
We know that to create truly effective change we need to study the healthcare system that patients journey through, and identify the reasons for the frustrations, delays and duplications that happen in everyday life. With this knowledge, we look to redesign how we all work together to provide joined up, person-centred care.
Our current projects include:
